কিটোটিফেন কি কাজ করে
কিটোটিফেন ট্যাবলেট- প্রতিটি পাতলা আবরণযুক্ত ট্যাবলেটে আছে কিটোটিফেন ১ মি.গ্রা . ( কিটোটিফেন ফিউমারেট বিপি হিসাবে ) । ওরোটিফেন সিরাপ ও প্রতি ৫ মি.লি. সিরাপে আছে ১ মিগ্রা . কিটোটিফেন কিটোটিফেন ফিউমারেট বিপি হিসাবে ) ।
টোফেন সিরাপ এর কাজ কি
কিটোটিফেন একটি শক্তিশালী অ্যান্টি অ্যালার্জিক ড্রাগ , যা প্রদাহ কারী মেডিয়েটরগুলোর কার্যকারিতা হ্রাস করে । কিটোটিফেন হিস্টামিন রিসেপ্টরগুলোকে প্রতিরোধ করে নন কম্পিটেটিড কার্যকারিতা প্রদর্শন করে । ফার্মাকোকাইনেটিকস কিটোটিফেন সেবনের পর প্রায় সম্পূর্ণরূপে এর শোষণ ঘটে । বায়োঅ্যাভাইবিলিটি এর পরিমাণ ৫০ % । কিটোটিফেন এর সর্বোচ্চ প্লাজমা ঘনতু ২-৪ ঘণ্টার মধ্যে অর্জিত হয় । এর প্রোটিন বাইন্ডিং ৭৫ % । কিটোটিফেনের অর্ধায় মাত্র ৩-৫ ঘণ্টা । মূত্রের সাথে প্রায় ১ % অপরিবর্তিত অবস্থায় এবং ৬০ -৭০ % মেটাবোলাইট আকারে ৪৮ ঘণ্টার মধ্যে নিষ্কাশিত হয় । মুত্রের মধ্যে অকার্যকরী কিটোটিফেন এন গ্লুকোরোনাইড প্রধান মেটাবোলাইট হিসেবে থাকে নির্দেশনা – হাঁপানির প্রতিরোধ মূলক চিকিৎসায় অ্যালার্জিক রাইনাটিস এবং কনজাংটিভাইটিস এর লক্ষণ যুক্ত চিকিৎসায়।
টোফেন সিরাপ খাওয়ার নিয়ম
টোফেন সিরাপ বা কিটোটিফেন ট্যাবলেট- পূর্ণবয়স্কঃ সাধারণত ১ মি.গ্রা . করে দিনে দুইবার খাবার এর সাথে খেতে হবে । বিশেষ ক্ষেত্রে এক সাথে ২ মি.গ্রা . করে দিনে দুইবার সেবন করা যাবে । ৩ বছরের অধিক বয়সের শিশু ১ মি.গ্রা . করে দিনে দুইবার খাবার এর সাথে খেতে হবে । বার্ধক্য – পূর্ণবয়স্কদের অনুরূপ মাত্রা অথবা চিকিৎসকের পরামর্শ অনুযায়ী । কিটোটিফেন সিরাপ : পূর্ণবয়স্কঃ সাধারণত ১ মি.গ্রা . ( ৫ মি.লি. ) করে দিনে দুইবার খাবার এর সাথে খেতে হবে । বিশেষ ক্ষেত্রে এক সাথে ২ মি.গ্রা . ( ১০ মি.লি. ) করে দিনে দুইবার সেবন করা যাবে । ৩ বছরের অধিক বয়সের শিশু : ১ মি.গ্রা . ( ৫ মি.লি ) করে দিনে দুইবার খাবার এর সাথে খেতে হবে । বার্ধক্য পূর্ণবয়স্কদের অনুরুপ মাত্রা অথবা চিকিৎসকের পরামর্শ অনুযায়ী ।
প্রতিনির্দেশনা কিটোটিফেন এর কোন উপাদানের প্রতি সংবেদনশীলতা আছে এরূপ রোগীদের দেয়া যাবেনা । ডায়াবেটিসের ওষুধের সাথে সেবন করলে সাময়িক ভাবে রক্তে অণুচক্রিকার স্বল্পতা দেখা দিতে পারে । অতএব এই দুই প্রকার ওষুধ একসঙ্গে সেবন থেকে বিরত থাকা উচিত।
সতর্কতা– কিটোটিফেন চিকিৎসায় খুব কম ক্ষেত্রে খিঁচুনি হওয়ার তথ্য পাওয়া গেছে । কিটোটিকেন ঘুমের ওষুধ , অ্যান্টি অ্যাপার্জিক ওষুধ এবং এলকোহল এর প্রতিক্রিয়া বাড়িয়ে দিতে পারে । মৃগী রোগীদের ক্ষেত্রে সতর্কতার সাথে কিটোটিফেন ব্যবহার করতে হবে ।
টোফেন সিরাপের এর পার্শ্বপ্রতিক্রিয়া
অ্যালার্জিক প্রতিক্রিয়ার কারণে ত্বকে মারাত্মক ক্রিয়া দেখা যেতে পারে । অতিরিক্ত সক্রিয়তা , নার্ভাস অথবা উত্তেজিত , ঘুমে বিঘ্ন হওয়া , সিস্টাইটিস , ওজন বৃদ্ধি প্রভৃতি পার্শ্বপ্রতিক্রিয়া দেখা যায় । চিকিৎসা শুরুর প্রথম কয়েকদিন তন্দ্রাচ্ছন্নতা এবং বিচ্ছিন্ন কিছু ক্ষেত্রে মুখ গহ্বরের শুষ্কতা , সামান্য মাথা ঘোরা ভাব হতে পারে । ওষুধ সেবনের কিছুদিন পর এইসব পার্শ্বপ্রতিক্রিয়া দূর হয়ে যায় ।
গর্ভাবস্থায় ও স্তন্যদানকালীন টোফেন এর ব্যবহার
কিটোটিফেন গর্ভাবস্থায় ও স্তন্যদানকালীন ব্যবহার যদিও গর্ভস্থ ভ্রুণের উপর কোন প্রকার বিরূপ প্রতিক্রিয়ার প্রমাণ পাওয়া যায়নি , তারপর ও গর্ভাবস্থায় কিটোটিফেন নেয়া থেকে বিরত থাকতে হবে । কিটোটিফেন মায়ের বুকের দুধের সাথে নিঃসরিত হয় । বলে স্তন্যদানকালীন ব্যবহার থেকে বিরত থাকতে হবে । সংরক্ষণ আলো থেকে দূরে , ঠান্ডা ও শুকনো স্থানে রাখুন ।

Ketotifen Composition
Ketotifen Tablet: Each film coated tablet contairis Ketotifen 1 mg (as Ketotifen Fumarate BP). Ketotifen Syrup: Each 5 ml syrup contains Ketotifen 1 mg (as Ketotifen Fumarate BP).
Ketotifen Pharmacodynamic properties
Ketotifen is a palent antiallergic drug which inhibits the effects of certain endogenous substances known to be inflammatory mediators. Ketotifen exerts a non-competitive blocking effect on histamine (H1) receptors.
Pharmacokinetic properties After oral administralion the absorption of Ketotifen is nearly complete. Bioavailability amounts to approximately 50% due to a first pass effect of about 50% in the liver Maxirmal plasma concentrations are reached within 2-4 hours. Protein binding is 75%. Ketotifen has a short half-life of 3-5 hours. In urine about 1% of the substance is excreted unchanged within 48 hours and 60-70% as metabolites. The main metabolite in the urine is the practically inactive ketotifen-N-glucuronide
Ketotifen Indications
Prophylactic trealment of bronchial asthma Symptomatic treatmerit of allergic conditions including rhinitis and conjunctivitis.
Ketotifen Dosage and Administration
Ketotifen Tablet: Adults :1 mg twice daily with food. If necessary the dose may be increased to 2 mg twice daily in severe cases. Children above 3 years: 1 mg twice dally with food. Use in elderly: Same as adult dose or as advised by the physician. Orotifen Syrup: Adults : 1 mg (5 ml) twice daily with food. IL necessary increased to 2 mg (10 ml) twice daily in severe cases.

Contraindication
the dose may be Contraindication Hypersensitivity to ketotifen or any of the excipients A reversible fall in the platelet count has been observed in a few patients receiving Ketotifen concomitantly with oral antidiabetic agent and it has been suggested that this combination should therefore be avoided,
Precautions
Convulsions have been reported very rarely during Ketotifen therapy. It shouid be used with caution in patients with a history of epilepsy. Ketotifen may potentiate the effects of sedatives, hypnotic, antihistamines and alcohol.
Slde Effects
A severe skin reaction can be caused due to allergic reaction. Feeling hyperactive, Irritable or nervous, disturbed sieep, Cystitis, Weight gain, feeling sleepy are the possible side effects. Drowsiness and in isolated cases, dry mouth and slight dizziness may occur at the beginning of treatment, but usually disappear spontaneously after a few days. Pregnancy and lactation Although there is no evidence of any teralogenic effect, Ketotifen in pregnancy and lactation is not recommended.
